Companies that provide mental health rehabilitation services focus on assisting individuals with maintaining community stability and independence in the most appropriate, least restrictive environment. We believe that clients desire more than maintaining community stabilization; they also want and deserve to live the life they truly desire. Most clients have developed learned helplessness from a perceived absence of control over their lives. Our focus is not only to help individuals become stabilized in the community but to also help them live the highest version of themselves through empowerment and self-actualization. We want individuals to realize their full potential. Our goal is to assist clients in understanding the importance we place on addressing the needs of the individual holistically, through integrated and evidence-based training, and the client-centered holistic approach we use in the provision of services. We believe that we can help clients become a creator as opposed to remaining a victim of their circumstances. We will help clients express their creativity, learn to be powerful, whole, resourceful and the writer of their own story. We are committed to motivating individuals to believe in themselves so that they may achieve the highest version of themselves.

SERVICES WE PROVIDE
We provide an array of Mental Health Rehabilitative Services to individuals with significant mental illness who are in need of client centered, goal-directed, solution based services in their natural environment to permit or enhance functioning in the community. We are committed to enhancing their quality of life by meeting and challenging the needs of the whole person: physical, mental, and social. We strongly believe that all individuals have goals and aspirations and a vision of their lives beyond their circumstances. In addition, we also believe that many of the individuals we serve may not believe that they have or are able to develop goals and aspirations that are obtainable. We assist our clients in developing a belief system that will empower them. Further, we provide them with creative resources and opportunities for growth while encouraging, motivating and training them. Our services integrate the principles of cultural competence, addressing the impact of discrimination/stigma, and inter-system collaboration into its service philosophy. We provide services with consideration of linguistic preference. Notably, an essential aspect of our services is recognizing the importance of family, community-based, and faith-based supports

HOW DO WE TRANSFORM HOPE INTO WELLNESS?
We believe that individuals who are experiencing feelings of hopelessness must go through 3 stages of growth in order to manifest a state of wellness. The role of our agency is to assist individuals, who are victims of circumstances, in their effort to transform their hopes into a state of wellness. This process is organized into three major ongoing and interacting stages/phases. The 3 phases of growth are:
HOPE
RECOVERY
DISCOVERY
The first stage is “Hope”. In this stage the client will receive assistance in developing a road map of the healing process, which, in conjunction with the worker, is accomplished with the active participation of the client, and whenever feasible, natural supports, friends and family members. In this stage emphasis will be placed on the following:

Exploring feelings of hope

Developing a client-centered treatment plan

Developing a Coordination of Care and Integration plan

Defuse or resolve the crisis situation

Provide linkage and training in using basic community resources
In the "Recovery" stage, In this stage the client will start to develop the tools necessary to become stabilize in his/her environment and start to see their potential unfold. They will start to make progress towards their goals and realize that the universe is friendly and the world is for them and not against them. The individual will:

Integrate and/or reintegrate into community life

Focus on stabilitization which includes; housing, employment, education, family involvement.

Reduction of harmful behaviors and adverse effects.

In the "Discovery" stage, the third and final stage, the clients will start to live a more purposeful life and actively participate in normal developmental life roles evidenced by:

COORDINATION OF CARE
Here at Circle Of Hope, we recognize the need to treat the whole person. This means not only will we focus on your behavioral health care needs, but for your physical and social needs as well. Circle Of Hope's staff will work with your Primary Care Physician, CSBs and other medical professional as needed to meet all your medical needs. The goal is for our clients to become as healthy as possible and for all of their care to be coordinated among all of their providers, internally here at Circle Of Hope as well as with their external providers. We will collaborate and share information among health care providers, who are involved with an individual's health care, to improve the care.
The purpose of Service Coordination is to ensure that the individual receives all needed services and supports; that these resources are well-coordinated and integrated; and that they are provided in the most effective and efficient manner possible.

Hope To Wellness Inc. is a nonprofit community based organization whose mission is to empower individuals to take control of their life, so they can live up to their full potential and live a healthier lifestyle. We provides an expansive array of mental health and wellness services with special emphasis on disadvantaged at-risk youth, adults with serious mental illness, veterans suffering from PTSD and teen age females who have been victims of sex trafficking. In this era of abnormally heighten mental health issues arising from nation-wide tragedies, our community based organization has set itself the task of still believing that we can transform hope into wellness by providing an array of compassionate innovative services and strategic activities to address the critical mental health and wellness issues facing youth and adults today.

In times of strife or uncertainty, it is easy to beat yourself up – to think you’re not good enough. It is even easier to forget that despite all of our differences we are born with one very important thing is common: potential – and lots of it. What seems difficult, at times, is remembering this potential for greatness lies within us all.
Tips for Teaching Functional Skills
Encourage the person to do as much as possible for themselves. Know each person’s goals and objectives and consistently implement formal and informal training.
“Teach” people rather than “do” for them. Be a good role model. Offer opportunities for choice. Look at every interaction as an opportunity to teach something
